Medical Referral*

Diagnostic examination by an affiliated licensed ophthalmologist when referred by a Co/op Optical Vision Designs optometrist for a previously undiagnosed condition. The referral must be the result of an eligible eye examination and must occur within 60 days of the original examination.

Lenses

First-quality prescription lenses in plastic, including:
…Single vision.
…Bifocal thru D-35.
…Tint - plastic lenses, Rose to 20%.

Frame

One frame retailing at $48, or a $48 allowance toward any other frame.

Contact Lens Services

$60 allowance toward a contact lens exam and contact lenses.
(in lieu of eyeglasses).

Employee, spouse, dependent children until their 19th birthday, and
