Medical Referral*

Diagnostic examination by an affiliated licensed ophthalmologist when referred by a Co/op Optical Vision Designs optometrist for a previously undiagnosed condition. The referral must be the result of an eligible eye examination and must occur within 60 days of the original examination.

Lenses

First-quality prescription lenses in plastic, including:
…Single vision.
…Bifocal thru Executive.
…Trifocal thru 7x28.
…Tint - plastic lenses, any single color.

Frame

One frame retailing at $68, or a $68 allowance toward any other frame.

Contact Lens Services

$100 allowance toward a contact lens exam and contact lenses
(in lieu of eyeglasses).
